Character Award Nomination Form

North Central Indiana Fellowship of Christian Athletes is proud to honor athletes from our area who demonstrate the four core values of FCA - Integrity - Serving - Teamwork - Excellence.

Guidelines for Nomination

1. Must be on a current team roster.
2. The winner does NOT have to be involved with FCA. We want to recognize young athletes who have great character and celebrate that with others in his/her community/school.
3. Nominations can come from the coach, athletic director, principal, athletic council, or any combination of the above.
4. Each school's honorees (one male and one female) will be selected by North Central Indiana FCA staff.
5. All nominees will be eligible for a $1,000 scholarship to be awarded during the FCA Banquet in May. There will be one male and one female chosen for a scholarship each year. Again, these individuals will be chosen by the North Central Indiana FCA office.
